FINAL YEAR GUIDELINE 2017. 



So, here comes Final Year. A year that is destined to make you “Dr.”.  Final year is fun. But it doesn’t mean you don’t have to study. In fact, In final year you have to study more. But in a different way. After passing all the four profs, you'll have enough idea what important and what not. Ti which stuff, you have to give your time..and which stuff doesn’t need any revision at all.
Here is some guideline.

Important note:

In final year you have to study whole year.:p Try to cover maximum syllabus during ward times. And give your ward test as if it’s a prof. this will eventually help you in prof prep. Do all the methods during your ward time. Present cases in wards, OPDs. Assist surgeries. Make presentations. Be clear about one thing. In final year you have to study yourself. No one will spoon feed you.  You have to learn yourself. You have to present cases yourself. Presenting cases in wards will  help you in preparing long cases in prof when you are given very short time. Rule is same in all. You may go in detail if you want. But always cover important topics first.

Emergency classes:


Learn maximum during emergency timings. Emergencies are best  for learning. Things you should learn during emergency morning/evening classes are :
  • Xrays.
  • Only important ECGS. Like MI, HYPERKALEMIA. A FIB.
  • Instruments  especially

·         OT instruments.
·         Bone tray.
·         vascular tray.
·         Laparotomy tray.
·         Iv cannula of different sizes.
·         NG stuff.
·         Foley stuff.
·         Different drains.
·         etc
  • Methods of sterilization especially in mayo.
  • Emergency surgeries. Especially  :

·         Appendectomy
·         Hernia surgeries
·         Amputation and debridment.

  • Trauma management.
  • Chest tube intubation.
  • Suturing.
  • Try to pass NG ,  FOLEY at least once.
  •  
  • Emergency treatment of diseases esp

·         MI
·         ASTHMA
·         COPD
·         ALLERGY
·         DIARRHEA
·         DKA
·         TETANUS
·         CVA
·         AKI
·         PULMONARY EDEMA
At your stage only these are important. If you want to go in detail , you can. But please don’t forget to cover important one first.


SURGERY

Surgery is not hawwa at all. Surgery is easy to study as compared to medicine. Bailey is not difficult, its easy to learn.
You should know detail account of following surgeries:
  • ·         Laparoscopic cholecystectomy.
  • ·         Herniotomy and hernia repair.
  • ·         Thyroid surgery
  • ·         Modified Radical Mastectomy.
  • ·         Appendectomy.
  • During ward study surgery as much as you can.

Topics to do must from Bailey:

Bailey is easy to study. Don’t go on its thickness/volume/breadth :P . it’s easy to memorize. Method to study Bailey is ; read whole chap and add your own points in summary boxes given at the end of each  topic.. Than you only have to revise those summary  boxes and tables.
  • Chap 2 Shock
  • Chap 4 skills (very imp )
  • Chap 5 surgical infections
  • Chap 6 surgery in tropics ( only imp topics )
  • PERIOPERATIVE CARE WHOLE UNIT
  • Especially  Chap 20 nutrition
  • Chap 22 day case surgery
  • TRAUMA WHOLE UNIT except chap 26 , 27.
  • Chap 35 – SPINE.
  • Chap 51. Thyroid
  • Vascular unit whole.
  • Chap 60. Hernia  whole.
  • Chap 64. Bariatric surgery.

You should read summary boxes of rest of the chapters after covering them from general dogar.
  • STERLIZATION IS THE ONLY CHAPTER PRESENT IN GENERAL DOGAR AND NOT in BAILEY.
  • Do systemic surgery from dogar.

Ward study:

Do norman browse if you have time. Or at least do differential diagnosis tables from it.
Rest you can do it from DOGAR OSCE latest edition. Its good book.

WATCH DAS VIDEOS FOR SURGICAL METHODS.THEY ARE EXTREMELY HELPFUL. NO NEED TO BUY DAS BOOK. YOU CAN TAKE THOSE VIDEOS FROM SENIORS, OR ELSE WATCH ON YOUTUBE.

MEDICINE

I did Kaplan medicine and Fischer’s lectures during our ward time. First watch Kaplan videos. Than study Davidson. Or whichever book you want to study. Don’t jump on Davidson without watching Kaplan videos. Please don’t study INAM DANISH or other local books. Davidson is a good book.
OXFORD MEDICINE IS GOOD FOR EMERGENCY TREATMENTS.
Try to do PRESENTING PROBLEMS , INVESTIGATIONS walay topics from Davidson just to have some good concepts. Also focus on tables of Davidson.

5Cs and their Infections from Davidson.

Endocrine achi tarah kr len. Coz it ll help you in surgery, paeds, gynae as well.

Bedside is for methods.

GEEKY. Com videos  on YouTube are best for methods

WATCH EMOREY ECG VIDEOS ON YOUTUBE FOR ECG. BUT YOU ONLY HAVE TO WATCH BASIC VIDEOS WHICH ARE NECESSARY. DON’T GO FOR ARRYTHMIAS, TORSADES LIKE STUFF (UNLESS INTERESTED)

Ward time :

Try to do as much methods during ward. Bedside is for methods
Try to pick findings. Findings will be ask in prof.
Always remember to do anthropometric measurements i.e HEIGHT , WEIGHT, OFCs during GPE. AND classify the child according to malnutrition . GPE IS VERY IMPORTATNT FOR PROF POINT OF VIEW.

GYNAECOLOGY/ OBSTETRICS:

We did Arshad Chohan and Kaplan.

But remember Kaplan mn kuch treatments unke country k according hain. So only grab concept from Kaplan is you want. Ante partum hemorrhage is very good in Kaplan.                                                   

During ward , in DR.  ARSHAD CHOHAN WARD ROTATION study labour on dummy and do instruments .  no need to wast time on models which are present in museum or Kemunited . Complete your maternity copy during ward and get them signed by all teachers in time, so that you should not worry about it in ward test or prof.
Try to enjoy maternity time. Its best time of the year .You can see or assist SVDS, C/S if you want.
